Here's an interesting one: failed crankcase pressure regulator which allows the crankcase to enter full vacuum. This causes air to be drawn in past seals and gaskets. In the day and age of power-adders (forced induction) such as turbo and superchargers, this can also allow FULL BOOST into the crankcase under heavy acceleration. Worst-case scenario, the pressure will blow out some very important seals or gaskets on the engine which will also have to be addressed.
