Ask another FQMS what that is, if they can't answer you don't want them to test your fuel.

This tank is out of compliance. It has several factors that have contaminated it's content. Because of years of neglect and no regular inspection ( one of the points of a Fuel Quality Maintenance Program) the fill basins have been damaged, as well as the fill tube cap and water is free to enter through an open vent tube. Poor oversight like this is why a fuel quality maintence company like Hughes fuel Recovery is so important to the maintenance of a criticle supply like the one in question at this health care facility.
