This is the result of improper wheel bearing maintenance/set.

All spindles had different greases in them, all of which appears to be the incorrect grease for the vault bearing setup they had. Replaced torsion arm/spindle assembly, new hub, all new bearings, and converted to Bearing Buddy setup.
