DATA PRIVACY CONSENT IN COMPLIANCE WITH THE DATA PRIVACY ACT OF 2012 AND ITS IMPLEMENTING RULES AND REGULATIONS
I allow Mazda Cebu to provide services in relation to the Mazda vehicle that I inquired about or purchased. Collect, use, disclose, and process my personal information set out in this form and/or otherwise provided by me or possessed by Mazda Cebu for the following purposes:
Sending marketing, advertising, and promotional information about other products or services that Mazda may be offering, conducting surveys for the vehicle(s) purchased and services performed by way of postal mail, fax, SMS, IG, Facebook Messenger, Viber, Whatsapp, Line, Telegram by way of voice call/phone call, and/or electronic transmission to my email address. Conducting feedback and comments for vehicle(s) purchased and services performed.
2. Retain my information for a period of five (5) years from the date of purchase of my vehicle or at such time that I submit to Mazda Cebu a written cancellation of this consent, whichever comes first. I agree that my personal information will be deleted/destroyed after this period.
3. Retain my personal information in Mazda Cebu database shared with third-party service providers; and

I am assured that security systems are employed to protect my information.
