📌 New Requirements for Student Permit ( at least 16 years old)

📍1. TDC Certificate ( MAPD Driving School)
📍2. Original & Photocopy of PSA Birth Certificate
📍3. Original & Photocopy of 1 Valid ID
📍4. Medical Certificate ( One Medical Clinic)
📍5. LTO Client ID ( LTMS Portal)
📍6. Accomplished ADL Form ( LTO)
📍7. Parent' s Consent ( for minor only)
📍8. Photocopy of Parent/ Guardian Valid ID
📍9. Photocopy of minor applicant Valid ID or any issued ID such as School ID and Certificate of Enrollment

📌 Requirements for Non Professional License:

📍1. Original Student Permit with Receipt
📍2. Medical Certificate ( One Medical)
📍3. PDC Certificate ( MAPD Driving School)
📍4. Accomplished ADL Form (LTO)

As of now March 28, 2022 still wala pa nag resume ang LTO SIPALAY sa full operation for SP and NON PROF processing as well as ONE MEDICAL CLINIC- SIPALAY man para SA MEDICAL EXAM and MEDICAL CERTIFICATE issuance.
